axis spin events

hi all,

an effort of educated guessing for those who have been around for a while: when an axis spins due to whatever cause, what percentage of cases ends in each of these scenarios?

a) no major damage (fork damage only)
b) motor damage
c) injured rider (wheel comes out causing accident or something like that)

I have read all of this and all of the endless sphere forum as well as parts of others.  From what I remember there have been a couple of fork only incidents but most involve fork damage and motor input wires get damaged from the spin after the fork gets loose. Seems that only a couple have had body damage their own that is.  One was pretty beat up and the other not so serious.
